You authorize charges to your Credit Card for lesson tuition. You will be charged the amount of the lesson tuition for that month the charge will appear on your Credit Card Account Statement.
You agree that no prior-notification will be provided and the amount will reflect the current month’s lesson tuition.
By submitting this form you agree to authorize Phil’s Guitar Shop to charge your Credit Card for any monthly lesson tuition.
If you select auto bill below your card will be charged on the 30th of every month reflecting the next month’s lesson tuition.
